Medicaid Nutrition Services Coverage Eligibility
Diabetes
Individuals diagnosed with Type 1, Type 2, or gestational diabetes are eligible for coverage for Diabetes Self-Management Education and Support (DSMES) with a written referral from their treating physician or a qualified non-physician practitioner.
Pediatric Populations
Children and adolescents from birth through 20 years of age (up until their 21st birthday) are covered for all nutrition services under the Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) program.
Note: Eligibility for coverage is subject to individual Medicaid plans and program requirements. Please verify with your provider or Medicaid office to confirm your eligibility and coverage specifics.
Medicare Nutrition Services Coverage Eligibility
Diabetes
Individuals diagnosed with Type 1 or Type 2 diabetes are eligible for Medical Nutrition Therapy (MNT) services and coverage for Diabetes Self-Management Education and Support (DSMES) with a written referral from their treating physician or a qualified non-physician practitioner.
Chronic Kidney Disease (CKD)
Coverage is available for beneficiaries with non-dialysis kidney disease.
Post-Kidney Transplant
Coverage is available for recipients of a kidney transplant within the past 36 months.

Your initial appointment is a 75-minute, in-depth nutrition assessment designed to give us a comprehensive understanding of your health, lifestyle, and nutrition needs. During this session, we take the time to explore:
✔ Your reason for seeking support – What brought you in and what you hope to achieve
✔ Nutrition-related symptoms & medical conditions – Identifying nutrition-related insufficiencies, excesses, and medical factors that impact your health and well-being.
✔ Eating habits & nutrient intake – Types, amounts, and timing of food, supplements, and medications
✔ Health history & lab results – Reviewing medical diagnoses, recent tests, vitals, and procedures
✔ Barriers & challenges – Identifying obstacles related to food, nutrition, and overall well-being
✔ Lifestyle factors – Sleep, stress, physical activity, self-care, and other health-related routines
✔ Food environment & accessibility – Exploring how availability and beliefs influence nutrition choices

In this first session, we take a comprehensive, personalized approach to understanding your unique relationship with food and health. Our goal is to create a supportive, non-judgmental space where you can candidly share your experiences. By assessing your nutrition patterns, lifestyle factors, symptoms, and personal goals, we begin developing a customized plan that supports your needs and integrates into your life

Follow-up appointments range from 30 minutes to 1 hour, depending on your needs. Since every individual is unique, there is no set number of sessions required—your plan is tailored to your progress and goals.
During follow-up sessions, we continuously assess and adjust your plan by evaluating:
✔ Your progress, identifying what’s working and what needs refinement
✔ How your body is responding, including any changes in symptoms
✔ Habit and behavior changes and their impact on your daily life
✔ Movement toward personal health and nutrition goals
