What Happens After You Refer

  • Patients can call, schedule online, or reach us through the website contact form. Providers may refer using our referral form, fax, or email, with details available on the referral page.

    We begin with a comprehensive assessment and develop a nutrition plan that aligns with the patient’s medical care and goals. Communication is tailored to provider preference. We typically share relevant notes with referring providers and coordinate more frequent contact for medically complex or higher acuity patients.

    Therapists and dietitians may also refer patients for outpatient meal support without transferring overall nutrition care. In these cases, we collaborate closely to understand the structure and support your patients need during meal support and share all related documentation.

    Our goal is to offer a nutrition space where patients receive consistent, high quality care and outside providers feel supported.
